Try using some Midi-QoL/DAE SRD sample Item, that apply an effect, like bless, and see if they work. If they work the problem is in the items you made, try using the sample items as a base to make yours.The best thing I can think of is under Workflow Settings you can set "Prompt Players to Roll Saves". You could set that to "Chat Message", and then your players could just ignore the prompt if they shouldn't be affected by the spell. Then it would roll all the NPCs automatically but not the players.
